Tax and Wage Adjustment Form (DE 678)

Use this form to get a refund on overpayment of income taxes after overestimating payroll. Payroll Tax Deposit Form (Form DE 88)

W-4

Used by the employee to calculate the amount of federal tax to be withheld each pay period. The W-4 is only a starting basis, as personal situations may dictate withholding more or less. In most cases, withholding corresponds to the number of dependents or exemptions claimed.


Form SS-4

This form is used to apply for the Employer Identification Number (EIN). Also known as a Federal Tax Identification Number (TIN), this number is used to identify a business, much like a social security number is used to identify an individual.


I-9

Also known as the Employment Eligibility Verification form, the I-9 must be completed by the employee and employer at the time of the employee’s hiring. The employer retains the form, intended to prove the employee’s legal eligibility for employment. Updated on 11/09/07.


Form W-9

The W-9 is used to acquire the taxpayer identification number (TIN) of an independent contractor or consultant. The form, which asks for an individual’s social security number or a contracting firm’s TIN, is used to certify that the individual or firm has provided an accurate TIN and that the party is not subject to backup withholding.
